Mental Health
Depression, anxiety, addiction, and other mental health issues are not a normal part of aging. Left untreated, they can lead to fatigue, illness, and even suicide. We promote programs that can help seniors cope. One in four older adults experiences some mental disorders such as depression, anxiety, and dementia. This number is expected to double to 15 million by 2030.
Our providers provide a depression self-management program designed to detect and reduce the severity of depressive symptoms in older adults with chronic conditions and functional limitations. It includes screening and assessment, education, referral to appropriate health professionals, and behavioral activation.
We offer our services at our provider office for most outpatient services, or in community, in-home, and telehealth.

Childhood Health
We work with stakeholders across the continuum of care to build integrated and innovative child health systems. Helping to ensure that children and adolescents stay healthy is very important to us--and important for their individual development! We know this is a combination of many things -- including a supportive family home, a thriving social network, and the possibilities of going to school. Their health is also affected by their feelings of safety when it comes to who they socialize with and feeling safe in the world around them -- the streets, in schools and on the playground.
Your child's health includes physical, mental, and social well-being. Most parents know the basics of keeping children healthy, like offering them healthy foods, making sure they get enough sleep and exercise and ensuring their safety.
It is also important for children to get regular checkups with their health care provider. These visits are a chance to check your child's development. They are also a good time to catch or prevent problems.
Other than checkups, school-age children should be seen for
